We have done our best to simplify the Florida & Duvall County permitting process. Below is a form to submit all necessary information. Please fill out in it’s entirety. You will need to attach your Government ID and BBP certificate(BBP must be from an approved source).
If you are already permitted to tattoo in Florida, upload that permit below. Those holding a Florida permit do not need to pay the $60 permitting fee that will appear after you complete the form.

These responses will be used for us to complete your artist permit.

Once your information is submitted a confirmation message will appear with a link to pay the $60 Artist Permit Fee.
